Your information is collected to ensure comprehensive medical treatment, health services and education can be provided. As a client of FHC we may collect demographic, health, medical, contact, insurance and payment information. All information is collected with your consent
If you are an employee of an employer who retains FHC to provide you with health services, we may collect personal information as well as information about your job. FHC may share aggregated statistics with your employer; however, your personal information cannot be identified from these aggregate statistics. We are committed to the privacy of your information and have safeguards in place to ensure anonymity and privacy.
FHC use of personal information
Information collected by FHC is used to assist in the delivery of our client care, programs and services. In addition, this information is also used to communicate with you regarding other FHC products, services, events, medical updates and educational information. FHC disclosure of personal information
Within FHC, access to information is strictly controlled. To assist us in providing a high level of care, and to ensure that you do not run out of your medication, please provide us with a minimum notice of 3 business days to process prescription refills. Once approved by your physician, the prescription can be faxed to your pharmacy. Please provide your Care Coordinator with the medication that you need refilled, your prescribed dosage and the pharmacy you would like us to send this to. Please note, there is a $40.00 fee for a prescription refill. This service is included as part of the CHAMP program.
Foothills Health Consultants is committed to sustaining healthy indoor air quality. In the interest of promoting the health and safety of FHC staff, clients and visitors, everyone is encouraged to maintain a scent-free environment. Our staff refrains from using perfumes, creams, cleaning supplies and other scented items to respect any allergies and sensitivities of our clients, other staff and visitors at all times. When you have an appointment with us, please refrain from using any scented products before your visit.
The ingredients in many fragrances and scents are known to irritate the respiratory tract, nervous system and eyes, lower immunity to disease, trigger allergies and other severe health reactions. In the case of asthma and epilepsy, reactions triggered by exposure to scented products can be life-threatening.
